HOW TO APPLY FOR A U.S. PASSPORT AND/OR PASSPORT CARD - DSP-11
1. PROOF OF U.S. CITIZENSHIP – State Certified Birth Certificate, Naturalization Certificate or expired U.S. Passport.
2. PROOF OF IDENTITY- Valid Driver’s License
3. PHOTOGRAPHS – 2 recent identical passport photos (2”x2”) in color with plain (white or off-white) background.
4. FEES – 16 yrs. of age & over: $110.00 (10 year) $30.00 (Passport Card)
Check or money order payable to: U.S. DEPARTMENT OF STATE
Plus $25.00 execution fee. Cash or check payable to: PROTHONOTARY
15 yrs or age & under: $80.00 (5 year) $15.00 (Passport Card)
Check or money order payable to: U. S. DEPARTMENT OF STATE

EFFECTIVE February 1, 2008: U.S. Passport applications for children under age 16 (15 & under) require written consent from both parents. Children applying with previous passports MUST also submit birth certificate showing the parent(s) names.
ALL CHILDREN MUST BE PRESENT WHEN APPLYING WITH PARENTS
The average processing time is within 25 business days from receipt by Passport Agency.
Applicants wishing to receive their passports sooner may request EXPEDITED service at an additional cost of $60.00 per application.
RENEWALS: (APPLICATION FOR PASSPORT BY MAIL) DSP-82
1. YOUR MOST RECENT U.S. PASSPORT – issued in your name within the past 15
years; and must have been issued after your 16th birthday.
