Fusion = Building Bonds

 The fusion formula is based on the idea of creating bonds. Bonds among clients, casemanagers and communities. Bonds among families, and even within our own organization.



Every piece of our TCDHS model is linked to create a seamless
system of accountability and interdependence:

  • A single administrator, who reports to the county commissioners, and two assistant administrators oversee the day-to-day operations of the Agency.
  • Our Family Services department provides case management duties for the Department.
  • All TCDHS case managers are cross-trained to provide holistic, family-centered services.
  • Our Common Application Form (CAF) means less paperwork for clients and speeds up the intake process.
  • Unified Case Notes provide a single source for client and family information.
  • A community-based Service Planning Team authorizes every client's service plan.
  • We provide a variety of services and programs through our own, County-run programs.
  • A special computer coding system allows us to track program expenditures daily.
  • Our Patch system of case management improves access to services, reduces travel time for staff and helps casemanagers build local support networks for families.
  • Cost-benefit ratio programming and performance-based contracts help us manage costs and ensure quality programs and services with area service providers.
